How to use this template
- Read the "Before you send" checklist below.
- Pick the variant that matches your situation (cooling-off, ETF dispute, end-of-contract non-renewal, or equipment-only return).
- Fill in the blanks.
- Send via the cancellation method specified in YOUR contract (most brands require certified mail). Keep a copy for your records.

Before you send
- [ ] Confirm the cancellation method in your contract. Phone-only, certified mail, in-app, or email — each brand specifies one (or more) accepted methods. Sending via the wrong method is treated as no notice.
- [ ] Confirm the notice window. 30 days, 60 days, 90 days. Send early.
- [ ] Have your account number, contract number, and contract start date in front of you.
- [ ] Send via certified mail with return receipt (even if email is also accepted). The receipt is your proof.
- [ ] Save a digital scan of the signed letter and the certified-mail receipt.
- [ ] Plan your equipment return logistics. Some brands require you to ship the equipment back; not doing so triggers a non-return fee.
VARIANT A — FTC Cooling-Off (within 3 business days of in-home sale)
Use this if you signed at home, at the door, or anywhere other than the seller's permanent place of business, AND the contract is for $25 or more, AND it has been 3 business days or fewer since you signed. Federal law (16 CFR Part 429) gives you a right to rescind.
